Insurance

On your first visit to our office we request that you bring with you an insurance claim form obtained from your employer along with your dental insurance card. We will gladly submit your claim to your insurance carrier for processing, however, the agreement of the insurance company to pay for your dental care is a contract between you and the company. Estimated co-payments, deductibles and any other balances not paid by the insurance carrier are the patients responsibility and due in full at the time services are rendered.
